• Swallowed or inhaled THC-containing products: Moderate. Benefit is modest. A 2026 review of chronic pain trials, mostly in nerve pain, found the synthetic THC drug nabilone reduced pain while dronabinol did not.
• CBD oil applied to the feet and legs: Limited. In a 4-week trial of 29 people, topical CBD reduced sharp, intense, cold, and itchy sensations without side effects.
• Swallowed CBD alone: Not established.

Compared with standard treatment: Gabapentin, pregabalin, duloxetine, and lidocaine patches remain first-line. Cannabis is usually added when these are not enough or not tolerated.

Good to know: Cannabis with gabapentin or pregabalin can add to drowsiness and dizziness.

What to ask for: A THC-containing oral product, balanced or THC-dominant, in the evening. For burning or tingling feet, add a CBD topical oil.
